Welcome to your patient centered medical home at the JYP-BMC Clinic, located at 780 Albany Street. What is a medical home? A medical home is our model of care where a team works with you to help address all your health care needs. Your team will include: your primary care provider (PCP), nurses, a medical assistant, a case manager, someone to assist with benefits and registration, and possibly, a behavioral health provider.
If you would like to become a patient at Boston Health Care for the Homeless Program, call 857-654-1605 to make an appointment. You can also visit us at 780 Albany Street to make an appointment or drop by our walk-in clinic, where you can get assistance signing up for health insurance and setting up appointments. The hours for our JYP-BMC Clinic are listed below.

- Tuesday 8:00am – 12:00pm & 4:00pm – 8:00pm
- Wednesday 8:00am – 5:00pm
- Thursday 8:00am – 8:00pm
- Friday 8:00am – 5:00pm

If you are staying at a shelter where BHCHP has a clinic present, you can also become a patient by checking in at that clinic. The days and hours of these shelter-based clinics are subject to change and the best way to make an appointment is to stop by the clinic and make an appointment in-person.
