CORI Information CORI (Criminal Offender Record Information)

Please be advised that in order to comply with Massachusetts Executive Office of Health and Human Services requirements, Boston Health Care for the Homeless Program conducts criminal record checks for all new employees. This check must be completed prior to beginning employment. A CORI form and CORI policy will be provided if you are offered employment. Thank you.

IMMUNIZATION RECORDS

All new hires are required to provide immunization or immunity documentation of measles/mumps/rubella(MMR) and chicken pox (varicella) prior to your start date. Tuberculosis (TB) testing can be provided during the first week of employment and bi-annual (every 6 months) testing is required. Hepatitis B vaccinations will be offered to new employess who are not already vaccinated.
