With over 1800 patients living with hepatitis C, a blood-borne infection that is the leading cause of liver cancer and liver transplantation in the United States, BHCHP was moved to establish the Hepatitis C Consult Service in 2014. These services, providing evaluation and management of new, highly tolerable and curative therapy for hepatitis C, are delivered at BHCHP by a multidisciplinary team of primary care providers, a nurse and a case manager with expertise in hepatitis C.
In the first 6 months of this new service, providers had evaluated over 180 patients and successfully started 55 patients on treatment. For patients who completed therapy, 94% achieved cure. The Hepatitis C Consult Service’s goal is to bring comprehensive, curative treatment to hepatitis C-infected patients at BHCHP within their existing primary care health home, reducing barriers to access and improving adherence to this crucial treatment.
