It can be confusing and time consuming for patients with complex health care, mental health, substance use, housing, and social services needs to work with numerous organizations in order to get connected to helpful services. We are introducing a new program to coordinate and improve care delivery across many organizations. If you would like to become a patient at Boston Health Care for the Homeless Program, call 857-654-1600 to make an appointment. You can also visit us at Jean Yawkey Place to make an appointment or drop by our walk-in clinic, where you can get assistance signing up for health insurance and set up appointments. You can also become a patient by checking in at one of our many clinics located in shelters throughout the city.
