Each year, we recognize the remarkable contributions of the Boston community in the lives of BHCHP’s patients. Please see below for more information on our three honorees. We can’t wait to celebrate them with you soon!

Change Maker Award – Connor Schoen, Co-Founder & Executive Director of Breaktime
Connor Schoen co-founded Breaktime in 2018 with fellow classmate Tony Shu to end young adult homelessness through employment and empowerment. While studying at Havard, he began volunteering at a local homeless shelter and was struck by how many young adults his age were experiencing homelessness. Breaktime has evolved from its early beginnings into a nationally recognized nonprofit with a lasting impact across Massachusetts and beyond. This past year, Breaktime transitioned from renting an office in Boston’s West End to purchasing its first permanent home at 63 Franklin Street in Downtown Crossing, establishing a lasting hub for innovation and collaboration to end young adult homelessness. Connor has been named to Forbes 30 Under 30 for Social Impact, and most recently was honored by WBUR with its inaugural Community Honors Award, recognizing five leaders shaping the future of Greater Boston through service. In his free time, Connor enjoys running, biking, and pickleball; in April 2025, he ran the 129th Boston Marathon for Boston Health Care for the Homeless Program.
Leader in Service Award – Boston Scientific
For many years, Boston Scientific has actively engaged with the BHCHP community through numerous service opportunities. Some examples include hosting Bingo for our respite patients, volunteering at the HER Saturday clinic, assembling care kits at company HQ, organizing sock drives, and so much more. We’re also honored to have three young professionals from the company currently serving on the Emerging Leaders Board.
Sox for Socks MVP Award – Pri-Med
Clean socks are one of the most requested essential items in our clinics. For 20 years, BHCHP has partnered with the Red Sox Foundation for an annual sock drive event at Fenway Park. We also invite local businesses and civic groups to take part in this challenge by hosting a sock drive at their workplace, and whoever collects the most socks receives the Sox for Socks MVP Award. This year, we present this award to Pri-Med for their record-breaking donation of over 6,000 socks!
