Coordinate all referrals for off-site, out-patient and on-site specialist appointments. This includes managing the new referrals and follow up specialist appointments, calling referral sites/providers to initiate or schedule appointments, submitting any needed documentation to referral sites, notifying patients of appointments, rescheduling appointments if needed, reminding patients of upcoming appointments.
•Coordinate referrals with other departments/programs/staff to facilitate patient transport or other needed supports to ensure appointment occurs.
•Thoroughly document referrals in EMR. Maintain tracking system for referrals.
•Communicate with specialists to identify their future clinic dates, notify Providers and nurses of these dates.
•Send out notifications to appropriate staff regarding opened and closed in-house specialist clinics.
•Ensure that all patient demographic, registration and insurance data is accurate in the EMR.
•Proactively problem-solve and resolve any patient insurance issues directly or by referring patient to the Benefit Coordinators.
•Contact review organizations and insurance companies to ensure prior approval requirements are met. Obtain and present necessary medical information such as history, diagnosis and prognosis. Provide specific medical information to financial services to maximize reimbursement.
•Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.
•Ensure that referrals are processed in a timely manner.
•Remind patients of scheduled appointments via phone.
•Assist administration in developing and monitoring quality improvement initiatives around referrals and related areas.
•Request and attach Behavioral Health authorizations to visits.
•Adapt referral tracking process as needed to support broader program initiatives (e.g. PCMH).
•Ensure that all reports from referrals have been received.

•One year experience in a related field or a clinical environment
•Strong computer skills, familiarity with MMIS, Epic Workque and NEHEN systems preferred Frequency of computer use at work: dailyKeyboarding: Proficiency required.

•Intermediate computer skills required
