Job summary: 

Boston Health Care for the Homeless (BHCHP) is seeking a Peer Recovery Coach to work with homeless individuals accessing our Office-Based Addiction Treatment (OBAT) services. At BHCHP, we believe in a world without stigma. We advocate for the expansion of access to all types of addiction treatment, and we also recognize a parallel need to reduce the harms associated with drug use for people who do not seek treatment, who cannot currently access treatment, or who have recently relapsed. We do not require our patients to stop using before we start helping them, and we know that recovery is a life-long process. The ideal candidate for this role is a compassionate and self-motivated individual who is himself or herself in recovery from substance use disorder, who embraces the mission of BHCHP, and who is committed to offering hope to homeless and unstably housed individuals as they work towards achieving sustained recovery.

Primary Duties and Responsibilities:

• Engages with patients identified with SUD, addressing barriers to successful recovery and serving as role model, ally, and advocate
• With team members, conducts initial needs assessment, reviews the patient’s SUD history, develops a wellness plan, and identifies the patient’s goals and expectations
• Works collaboratively with treatment team to implement evidence-based services to patients with SUD
• Conducts active outreach efforts to encourage attendance and participation in recovery-oriented, self-help, and pro-social groups
• Educates patients about addiction and utilizes motivational enhancement techniques to explore ambiguity and tip the scales toward treatment
• Provides peer coaching and supports patients in establishing treatment goals and achieving and maintaining recovery via multiple pathways
• Is available to provide support to patients in immediate risk of relapse or for other time-sensitive matters during and outside of regular business hours
• Helps patient navigate the healthcare system and social service system; accompanies patients to appointments and meetings as needed
• Assists with access to treatment for SUD and co-occurring conditions, creating systems and procedures to rapidly identify treatment needs and determine appropriate level of care. Assists with data collection for quality assurance and program evaluation
• Meets patients with SUD prior to discharge from facilities and facilitates patient access to next stage of treatment
• Serves as a liaison between the recovery community, the addiction treatment system, the medical treatment system, and the patient’s community, family, and social context to facilitate connections across systems of care
• May facilitate and/or co-facilitate weekly recovery support group(s)

• Clearly and effectively facilitates communication between the patient, PCP, psychiatrist, and any external providers maintaining appropriate confidentiality procedures and boundaries
• Maintains a professional approach
• Maintains accurate and up-to-date (non-clinical) records and standardized data on all patients in Epic, the medical record system

Team Based Collaboration:
• Functions as part of a team that includes psychologists, physicians, social workers, nurses, and case managers
• Systematically reviews the caseload with the supervisor each week, focusing on new patients and patients who are having difficulties
• Participates in scheduled/required recovery coach mentorship and training activities and meetings with Massachusetts General Hospital and other surrounding hospital collaborations

Qualifications and Requirements: 

• High school diploma required
• Minimum of 2 years in sustained recovery at time of application
• Recovery Coach Academy graduate preferred or willingness to complete RCA within first year of employment.
• Willingness to obtain Recovery coach CARC certification preferred within first 12 months of employment
• Bilingual candidates encouraged to apply

• Strong interpersonal, facilitation, and leadership skills
• Interest and experience working with underserved populations
• Ability to work independently as well as part of a multidisciplinary team of psychologists, physicians, social workers, and nurses
• Strong advocacy skills along with knowledge of community based services, resources, and local recovery community
• Comfort with multiple pathways to recovery from SUD and willingness to embrace a patient-centered approach that recognizes an individual’s preferences and autonomy (“recovery by any means necessary”)
• Knowledge of harm reduction strategies including opioid overdose prevention and response, safer injection techniques, or willingness to be trained to become expert in these topics
• Skills in motivational enhancement strategies preferred
• Excels at problem solving and multi-tasking; organized, efficient and goal-directed
• Effective use of the Internet and Microsoft Office programs such as Word and Outlook
• Effective use of Microsoft Excel and PowerPoint a plus
• Ability to handle confidential information

Physical and Mental Requirements
• Ability to lift and carry up to 10lbs on a regular basis.
• Standing and walking with peers during mobile outreach on foot in the neighborhood
• Phone use
