Family Outreach Services

Families are the fastest growing segment of the homeless population. For 20 years, BHCHP's Family Team has been a stable source of care for homeless families at more than 25 family and domestic violence shelters across greater Boston.

BHCHP's Family Team provides:

• Quality health care delivered during weekly visits to shelters;
• Mental health and substance abuse counseling;
• Educational, recreational, and therapeutic programs for children;
• On-site case management services to connect families with needed social services; and
• Assistance with disability and medical insurance benefits.

Every year, BHCHP's Family Team cares for nearly 1,800 homeless parents and children.
