Family Outreach Services

Families are the fastest growing segment of the homeless population. For 24 years, BHCHP's Family Team has been a stable source of care for homeless families and is now the medical presence in more than 30 family and domestic violence shelters across greater Boston and in the motels that are used when shelters are full.

Safe and Sound A Health Child Toolkit for Homeless Mothers

BHCHP's Family Team provides:

• Quality health care delivered during weekly visits to shelters;
• Mental health and substance abuse counseling;
• Educational, recreational, and therapeutic programs for children;
• On-site case management services to connect families with needed social services; and
• Assistance with disability and medical insurance benefits.

Every year, BHCHP's Family Team cares for nearly 1,800 homeless parents and children.
